Services Provided

The aim of services are to identify and assist the identified child and his/her family in resolving conflicts; coordinate efforts between the counselor, the child and family, schools, the Department of Family and Children Services and the Department of Juvenile Justice system in order to maintain the child in the home and protect the community; communicate and demonstrate methods of appropriate parenting skills and/or behavior management techniques in order to help family members more effectively manage certain behaviors, and/or support and strengthen the identified child’s home environment.

The primary type of service provided by Family and Community Partners is Intensive Family Intervention services such as: crisis management, intensive case management, skills training, individual and/or family therapy and other rehabilitative supports to prevent the need for a out of home placement or more restrictive services.

Our services include:

1.  In–home services, which include wrap–around services to the consumer, parents, or      other caregivers regarding behavior management skills, etc.

2.  Address concrete therapeutic and environmental issues.

3.  Services are family focused, delivered in the consumer’s home or other location in      the community.

4.  The majority of the services (60% or more) are provided face–to–face with      consumers and their families and eighty percent (80%) of all face–to–face services      are delivered in non–clinic settings.

5.  A team approach to consumers that will encompass services in the home, schools,      detention centers, the community, etc.

6.  Services are available 24 hours a day 7 days a week for consumers and families who      are experiencing crisis.

Family and Community Partner’s services shall be provided for the identified child based on the assessed needs. The purpose of the services is to reinforce and enhance the identified child’s ability to function within his/her school and home environment and to enhance the family’s level of functioning.
